2. Must have identification ready for provider to check upon first visit with us ( NO EXCEPTIONS)

3. it takes approximatly 24-48 hrs to get approved so please bear with us and we will try and do it faster.
4. Upon verifying information given we will send an email to you stating you have been accepted into our family and you will receive a client number.
5. No new clients on weekends will be accepted. Regulars only
