Things you should know in order to join our happy family..
1.It is imperative that you fill out this form in it's entirety. Put N/A where you do not have something such as handles or Date Check ID. All forms not filled out to our specifications will be deleted.
2.Must have identification ready for provider to check upon first visit with us ( NO EXCEPTIONS)
3. It takes approximatly 24-48 hrs to get approved so please bear with us.
4. Upon verifying information given we will send an email to you stating you have been accepted into our family
and you will receive a client number.
5. No new clients on weekends will be accepted. Regulars only.
YOU MUST HAVE A ID # AND HAVE USED OUR SERVICE @ LEAST THREE TIMES!
